Some depictions of Tiamat show her as a multiple-headed dragon, but she usually has 5 heads.

Are you perhaps thinking of a chimera? It has a lion's head, a goat's head, a dragon's head, the body of a lion and the tail of a scorpion. Some of them also have the wings of a dragon.
